MEMBERSHIP
Completion of our simple registration form enables prompt fulfillment of our member's instructions at the time of death. Membership advantages are for those who wish to pre-plan. A membership card is issued and a permanent file is maintained at our office. This is a one time non-refundable fee. This applies to those who are of sound mind and have no known acute illness(es).
Membership fee for an individual - $25.00
Membership fee for a couple - $40.00
DIRECT CREMATION
This fee includes the transportation of the deceased to Monarch within a 15 mile radius (additional charges will occur beyond the 15 mile radius and/or from a residence), a minimal cremation container, cremation, filing necessary permits, and *professional services.
Members from - $1,100.00
Non-Members from - $1,250.00
SIMPLE CEMETERY BURIAL
For those who prefer simplicity, an alternative to cremation is a simple burial which does not require embalming, as performed in early America. This fee includes transportation of remains to Monarch within a 15 mile radius (additional charges will occur beyond the 15 mile radius and/or from a residence), care and holding of remains, transportation to a local cemetery, filing necessary permits, a simple casket, and *professional services. (Please note: simple cemetery burial requires that at no time shall the casket be opened after leaving Monarch Society's premises).

TRADITIONAL FUNERAL SERVICE AND BURIAL
Funeral Service may take place at Monarch Society, church or other location in the local area. This fee includes transportation of remains to Monarch within a 15 mile radius (additional charges will occur beyond the 15 mile radius and/or from a residence), transportation to local service location, then to local cemetery, filing necessary permits, *professional services, and necessary preparation of remains including embalming, dressing and cosmetology. (Additional fees will be charged for special care of autopsied remains or for reconstruction/restoration when necessary).

TRADITIONAL FUNERAL SERVICE WITH CREMATION TO FOLLOW
Funeral Service may take place at Monarch Society, church or other location in the local area. This fee includes transportation of remains to Monarch within a 15 mile radius (additional charges will occur beyond the 15 mile radius and/or from a residence), transportation to local service location, then back to Monarch, filing necessary permits, *professional services, and cremation. (If open casket viewing is desired there will be additional charges for preparation of the remains from $800.00).

*PROFESSIONAL SERVICES
Our professional service fee is included in ALL our services. This fee includes: personnel available 24 hours a day to respond to initial call, arrangement conference and counseling services, preparation and filing of necessary legal documentation for death and final disposition, notification of death to Social Security, filing for Veterans benefits (if applicable), staff assistance and coordination of service plans. Also covers overhead fees such as facility maintenance, equipment and inventory costs, insurance and administration expenses, and governmental compliance.

SERVICES AT MONARCH SOCIETY
We can accommodate specific services in our beautiful Victorian home with seating available for up to 30 people. (Additional charges for after hours, weekends, and holidays).
As an alternative, families may hold a memorial service at any location that has special meaning. For example, your home, your place of worship, a park, in the mountains, etc.

SHIPPING OF CREMATED REMAINS
By request, we can ship cremated remains anywhere. This fee includes preparation and registered mailing through the US Postal Service.

SHIPPING REMAINS OUT OF STATE/COUNTRY
We can arrange for the shipping of embalmed remains anywhere in the world. The fee depends on costs and services desired and will be quoted by our staff.
PAYMENT OF FEES
Fees are due at the time services are rendered. Prompt payment permits Monarch to continue to offer services at the lowest possible cost. Fees are subject to change without prior notice. We serve members and non-members alike.
THE MONARCH INSURANCE PLAN (PRE-PAYMENT)
Monarch offers a funeral insurance plan whereby members may purchase a policy to help defray or fully cover the cost of services. Information concerning the insurance plan may be obtained from our office.
